Some Key Responsibilities:

Provides welcoming, engaging, and accessible services addressing physical health care, behavioral health care, and social needs with a recovery-focused, strengths-based approach.

Promotes a warm and engaging organizational culture aligned with NorthCare values and trauma-informed care principles.

Demonstrates a strong understanding of peer recovery and wellness coaching practices, supporting clients with complex needs using evidence-based interventions and resources.

Works as part of a multi-disciplinary team to support health behavior change in individuals with behavioral health challenges, fostering genuine and professional interactions with patients, families, and community partners.

Conducts health promotion services individually and in group settings, including monitoring vitals, providing education, developing self-management plans, and facilitating lifestyle interventions.

Advocates for clients within NorthCare and the community, assisting in recovery and independent living processes, facilitating peer support groups, and supporting the development of self-advocacy skills.

Complete assessments, develop care management plans, and track outcome measures for clients to integrate physical and behavioral healthcare needs effectively.

Maintains professional competency through ongoing education, training, and compliance with certification requirements and agency policies.

Meets ODMHSAS criteria for Peer Recovery Support Specialist and obtain required certifications within specified timeframes.

Minimum of one year experience in a mental health or related field.

Proficiency in MS Word processing software, intermediate computer skills, and excellent verbal and written communication skills.

High degree of flexibility and efficiency in performing job responsibilities.

Access to reliable transportation and ability to travel locally as needed.

Maintains CPR, First Aid, and other required certifications, with willingness to obtain additional training as necessary.
