Job Description: 

Are you an experienced caregiver with a passion for supporting children, youth, and families? NorthCare is seeking a Level 2 Family Support Provider to join our dedicated team. In this role, you'll play a crucial part in advocating for families, providing essential treatment and support, and ensuring they receive the care they need. Essential Job Functions:

Support the implementation of unified systems of care, guiding families toward long-term stability.

Provide coaching, modeling, and support to empower individuals and families, helping them find strength and resources.

Respect the integrity and confidentiality of individuals receiving services.

Foster the development and strengthening of natural supports within the community.

Actively participate in treatment with child, youth, and family teams, aiming to keep them in the home and avoid out-of-home placements.

Empower individuals and families through modeling, advocacy, coaching, and support.

Develop and maintain positive relationships with community partners, serving as a liaison for NorthCare.

Manage complex cases, coordinating services through the wraparound process.

Maintain accurate records and documentation in compliance with requirements.

Meet patient contact/caseload standards as established by the Director.

Ensure prompt communication via phone, text, and email.

Maintain consistent work attendance and promptly communicate any late arrivals or absences.

Participate in meetings, consultations, and training sessions to expand your knowledge.

Provide services in the community, including clients' homes, local venues, or via telehealth.

Transport patients to various locations in the community and provide services in the home.

Qualifications

High School diploma or equivalent.

Lived experience as a caregiver navigating mental health services or similar systems of care.

Obtain ODMHSAS Family Support Provider certification within 60 days of hire.

Obtain ODMHSAS Wellness Coach Certification within 60 days of hire.

Minimum of one (1) year experience required in a community mental health or mental health-related field.

Proficiency in MS Word and intermediate computer skills.

Strong interpersonal and communication skills.

Access to reliable transportation for local travel.

Valid Oklahoma driver’s license and automobile liability insurance.

Obtain CPR, First Aid, and Therapeutic Options certifications within 30 days of employment.

Complete all NorthCare required training within 30 days of employment.

TB test within 12 months prior to employment or obtain within 30 days of employment.

Applicants with a bachelor’s degree OR three (3) years’ experience in the community mental health field may be considered for Level 3 Family Support Provider.
