Family Support Provider Home and Community Based Level 1

Essential Job Functions:
Support the implementation of unified systems of care, working towards long-term stability for children, youth, and families.
Provide coaching and modeling to empower individuals and families, helping them find strength and resources.
Respect the integrity and confidentiality of those receiving services.
Foster the development and strengthening of natural supports within the community.
Actively participate in treatment with child, youth, and family teams, aiming to keep them in the home and avoid out-of-home placements.
Empower individuals and families through modeling, advocacy, coaching, and support.
Develop and maintain positive relationships with community partners, serving as a liaison for NorthCare.
Manage complex cases, coordinating services through the wraparound process.
Maintain accurate records and documentation in compliance with requirements.
Meet patient contact/caseload standards as established by the Director.
Ensure prompt communication via phone, text, and email.
Maintain consistent work attendance and promptly communicate any late arrivals or absences.
Participate in meetings, consultations, and training sessions to expand your knowledge.
Provide services in the community, including clients' homes, local venues, or via telehealth.
